1.3. II. Organization of reception

2.1. 4.7. Upon admission to training under the main professional educational program of secondary vocational education, applications are accepted from persons with a state-owned document on average (full) common education, basic general education, primary vocational education, secondary vocational education or higher professional education.

4.8. The applicant applies the following information:

2) date and place of birth;

3) details of the document certifying his identity when and by anyone issued;

4) place of residence;

5) information about the previous level of education and the formation document, its confirming;

6) a specialty (s), for training on which he plans to enter an educational institution, indicating the form of education and training conditions (budget places, places under contracts with the cost of training);

7) information about the delivery of a unified state exam and its results or on the place of delivery of a single state exam in additional terms of the unified state exam (if there are several results of a single state exam, the validity of which has not expired, the incoming point in the statement, what the results of the Unified State exam and for some general education items it uses);

8) the presence / lack of a winner diploma or a winner of the corresponding schoolchildren's Olympics (if available - indicating the name of the Olympics, the details of the winner's diploma or the winner of this Olympiad);

9) the presence / lack of special rights upon admission to the educational institution established by the legislation of the Russian Federation (if available - with the indication of such rights and information on the document confirming the presence of such a right);

10) The need to provide a hostel.

4.10.
